Your Child’s Medical Record at Freshfield in relation to Asthma

Dear Parents/Carers

RE: Your Child’s Medical Record at Freshfield in relation to Asthma 

We are writing to you because our records show that you have informed Freshfield Nursery that your child has asthma. If this is no longer the case please contact the school office to let us know that your child no longer has a diagnosis for Asthma. For all children with an inhaler in school we require a parent/carer to complete a medication permission & record form for your child. We are unable to administer any medication at nursery without this form being completed beforehand. If you require a copy of this form to be filled in please call in to the school office or alternatively telephone the school office on 0161 442 6944 so that we can email the form to you so that you can complete it and email it back to us.

As we approach the winter months sometimes the cold air and winter childhood illnesses can affect a child who is asthmatic, and can exasperate their asthma condition. Therefore if your child has been given a diagnosis by a doctor or the hospital of an asthma related condition please ensure that nursery is always updated with any new relevant information.

To ensure that we can always prioritise your child’s health needs, please can we request that you telephone our school office on 0161 442 6944 or send an email to our school office at admin@freshfield.stockport.sch.uk if your child is ever hospitalised as a result of their asthma.

Following a significant asthma attack which required medical intervention, it is very important that you speak to us at Freshfield, prior to your child’s return to nursery. This is in order to have a medical reintegration conversation. This conversation is vital to inform nursery staff of any changes to medication and if any additional monitoring is required by us at nursery.

To update nursery following your child’s hospitalisation, please speak with the EYFS Leader and Inclusion Leader Miss Simcock or one of the Co-Headteachers in person, or alternatively telephone or email us to update us accordingly, especially if your child’s medication has been amended. We will then update our records accordingly, and circulate any necessary information to our staff team.
